Information and Certifications

I hereby authorize Tradebank International, Inc to charge all my Tradebank fees due to the EasyPay method of payment provided above on the 10th of each month. I understand that by signing this form I am assuming responsibility to verify the amount that will be processed as indicated on my Tradebank account prior to the 10th of each month. I may obtain this amount from my monthly statement provided online via myTradebank.com on the 2nd day of each month. I understand this form will be valid until I cancel this authorization with a written notice submitted to Tradebank: (Attention EasyPay: 225 Reformation Pkwy, STE 200, Canton, GA 30114). To implement any changes or to terminate my EasyPay agreement, I must provide a 10-day written notification to Tradebank International, Inc. I understand if my EasyPay information is updated, and there is a balance due, the amount will be debited the same day. Any discounts associated with EasyPay will become effective on the day in which the request is processed. I understand that any discount(s) associated with my Tradebank account are contingent upon successful processing of payment and that consecutive failures to obtain authorizations with the account information I have furnished above may result in reclassification of my account. I acknowledge that any payments declined or returned from my financial institution will be assessed a non-refundable $25 administrative processing fee.

By typing your name in below, you are signing this Agreement electronically. You agree your electronic signature is the legal equivalent of your manual signature on this Agreement. You consent to be legally bound by this Agreement's terms and conditions. You further agree that your use of a keypad, mouse or other device to select an item, button, icon or similar act/action, or to otherwise provide Tradebank International, Inc. or in accessing or making any transaction regarding any agreement, acknowledgement, consent terms, disclosures or conditions constitutes your signature (hereafter referred to as "E-Signature"), acceptance and agreement as if actually signed by you in writing.
